The Specifics of Nursing Care for Patients after Resection of the Tongue on Oral Suregery Department

This thesis deal swith the specifics of nursing care for patiens with a resected tongue in the oral surgery department. The objectives of this work include focusing on the basics of care for patiens after a tongue resection and the most common problems nurses may encounter hen caring for these patients. The work consists of two parts: the theoretical and empirici parts. The theoretical part describes the field of dentistry and oral surgery, anatomy and physiology of the oral cavity. It also deals with orofacial surgery and the actual tongue resection procedure with a focus on nursing care for patiens after wards. In other chapters of the theoretical part we can read about the most common nursing problems in patiens after a tongue resection. In addition, the twelve patterns of health in Marjory Gordon's nursing model will be described for the empirical section. The empirical part is devoted to the research investigation. The thesis describes the metodology and organization of the research investigation. This was a qualitative research, the method of data collection was to create three case reports or case studies with follow discussion. Intensive care of patient with metformin intoxication

Metformin is the first-line oral antidiabetic drug for patients with type 2 diabetes mellitus. As the rate of new diagnosed cases of diabetes increases year-on-year, the number of patients who are prescribed metformin rises as well. In the Czech Republic only, 858,010 patients were treated for diabetes in 2015. 786,586 of them were treated for the type 2 diabetes mellitus. Metformin intoxication, called also MALA (metformin- associated lactic acidosis) after its main symptom, is a rare complication (≤ 10 cases per 100,000 patients per year) in the metformin treatment, but the mortality is high (up to 50%). Metformin-induced lactic acidosis develops when the contraindications of metformin therapy are not respected or in the event of an acute disease linked with dehydration and hypoxia. Patients with MALA are hospitalized at the int ernal medicine intensive care unit or the department of anesthesiology and critical care with multiple organ failure. This thesis aims at determining whether procedural nursing care standards, implemented in the form of a nursing process, are actively used in practice on intensive care beds. The prevention of nosocomial infections at central venous catheters in standard inpatient departments and intensive care units.

Nosocomial infections are one of serious problems of modern medicine and nursing also nosocomial infections mean real serious problems for future. Prevention of nosocomial infections, applied together with pointed and meaningful antimicrobial therapy is the only causal solution now. This work was aimed primarily at a comparison of knowledge of common nurses working in intensive care wards and common nurses in standard wards. In general, it could be said that the aim of this work was to map the scope of knowledge of common nurses as for nosocomial infections as well as aseptic care for central venous catheters including catheter sepses. The found deficit in knowledge of common nurses pointed out the occurrence of mistakes in nursing practices. Quality of nursing is connected closely with standardisation of nursing procedures. Quantitative research inquiry was realised in 4 medical facilities: Regional hospital Liberec, a.s., Masaryk city hospital in Jilemnice, Hospital České Budějovice, a.s. and Hospital with polyclinic in Semily. As the research method there was chosen a non-standardised questionnaire. The sample of respondents was chosen as an intentional selectionand and in to statistic processing included 290 relevantly filled out forms. The research investigation also included a comparison and quantification of nursing standards. In view of the performed comparison of standards in nursing we concluded that creation of incomplete or even inconsistent standards occurs here. We have prepared a "Proposal on a standard of nursing care for inserted venous catheter", of which form could enable to implement it in medical care facilities within the Czech Republic and define clearly basic nursing procedures within the scope of these problems.
Healthy aging in the context of nursing care

Healthy Ageing Programme is a concept which should have a significant influence on every resident in the countries where population median age has been quickly increasing. This topic of healthy ageing is the key issue of the given Master´s Diploma Thesis. At the present, our country is involved not only in several smaller-scale regional programmes focused on lives of seniors but also in the events of European importance. If we want to provide our seniors with a quality life in all aspects, it is necessary to encourage promoting Healthy Ageing Programme. The wide scope of activity is thus open, especially for nurses. The first objective develops the approach of seniors in the selected health-care facilities to healthy ageing. The second objective introduces the approach of a client before retirement age to the healthy ageing principles. Finally, the third objective specifies the role of general practice nurses in their support of clients before retirement age in their healthy ageing. Research questions included the following issues: the approach of a client at the age before retirement to the healthy ageing principles and the role of a general practice nurse and her support of the abovementioned clients. The given research also develops one hypothesis regarding the fact that personal objectives will change depending on barriers.Both qualitative and quantitative research was used in the given Diploma Thesis. An in-depth interview was conducted during the survey. Additional information was obtained on the basis of an opinion poll. Two research groups were used for the interview: the first one consisted of 11 respondents from the general practitioner´s outpation departments, the second research group consisted of 6 nurses working for the aforementioned departments. The total number of 150 respondents from the designated health-care facilities was selected for the survey during which they were answering the research questions. As a result, a large number of useful information was obtained. The clients' approach can be divided into several groups: leisure activities, education, workload, prevention and medical examinations, regimen, community nursing care, healthy ageing. The key roles of a nurse should take into consideration the following aspects: healthy ageing from the nurse´s point of view, prevention and geriatric nurse at the outpatient department, community nursing care, health support of the patients provided by a nurse. Information obtained from the interviews were interconnected and often corresponded with each other. Research questions are also followed by the given results. A nurse has a significant role in the area of education, provision of necessary information to the clients, promotion of prevention programmes informing the clients about the solution to their problems. The motivation of clients before retirement age is also crucial because it is at this age when people can influence their life, its length and quality. The survey also observes the general perception of people of an old age and changes in their point of view with age. The information collected from this survey may be also used in different studies in order to improve both quality of nursing care and motivation of the clients before their retirement.
